Documents Required by Labrador Life Line to complete your Application

Once your case has been assigned a case worker, we will request that the following documents be faxed and/or emailed to us

 

Estimate for Procedure

Get a written estimate from the veterinarian who will be treating your pet for its current condition. The estimate must be on your veterinarian's letterhead and include the following information:
  • Your name and the name of your pet
  • A complete breakdown of costs for treatment.

Letter of approval or denial from Care Credit

You are required to apply for Care Credit if it is accepted by your veterinarian.
If you applied for Care Credit, Labrador Life Line needs a copy of the letter of approval or denial.
If you apply online Care Credit will provide you with an immediate response. You can do a screen print or copy and paste the response which you can fax or email to your case worker

Lab Rescues only Labrador Life Line will need copies of the following:
  1. Labrador Life Line Application for Rescue Identification
  2. Copy of your adoption application.
  3. Copy of your adoption contract.
  4. Reference from your veterinarian.
  5. References from at least two people who have adopted from your organization (1 must be your veterinarian)
